the actual most painful thing you can do in a level is be good enough at games to verify what is called a "masochist" level by only me, common levels like this include my level Inception, Glitchy dive test, and a level again but that got free roamed. I would say the most painful levels dont come out of normal levels. but rather badly designed event levels. as you feel as if you are required to do it or you need to do it. making anger alot more. dont make actual painful levels because nobody will like you and will likely cheese it if you dont know anything.

see ultra kaizo levels are the best answer because they physically hurt your hands to play

I will also say if your going for a Painful level it is better to go for more of a funny troll llevel angle, Troll level psychology isnt pain, but rather failing at something in a funny way and having to think of unique ways to counteract it, thats what keeps it engaging, not pain, nobody likes pain, its the funny.
